Privacy

The PPG consists of a number of patients who meet regularly, and share information through social media, irregular newsletters and this web site. The group holds information on Practice patients who are members of the PPG.

The information can only be accessed by the Chair of the PPG and Membership Secretary, together with members of the practice administration team, who have been tasked with verifying you as a patient of the Practice.

Who might we share your information with?

The information is not shared with anyone else.

What information do we collect from you?

• PPG membership details used to communicate with patients - name, email address and telephone number. This information is retained and held securely
  
  
• Additional PPG membership details used to verify the applicant as a Practice patient - date of birth and post code. Once the applicant is verified as a patient, this additional information is deleted.

Your individual rights!

You have the following rights with regard to your data:

• The right of access to your personal data which the group holds about you.
• The right to be forgotten (erased) - to request your personal data is erased where it is no longer necessary for the group to retain such data.
• The right to withdraw your consent at any time.
• The right to rectification (correction) - to request that the PPG corrects any personal data if it is found to be inaccurate or out of date.
